It was first used by Democrats to describe the many FAKE News stories about HRC, like the ones about her running a child sex trafficking ring or the stories about her dying from a variety of illnesses.
There was even a FAKE news story about the Pope endorsing Trump.

But the term has now been co-opted by the right to describe any news that they don't like.

Here is what I have pieced together so far.

A 35 page memo that has been floating around for a while found it's way to John McCain who turned it over to the FBI.

A two page summary of that memo was attached to the briefing given to Trump, Obama and a gang of 8 members of Congress.

That fact was leaked to Reuters and reported by CNN.

However, here's what the person who leaked the story to Reuters seems to have left out.

That two page summary was marked 'disinformation' and was only being briefed to inform Trump (and the others) of the type of stuff that was floating around out there that they felt that he should be aware of.

By leaving that fact out, either intentionally or not, it made it look like the two page summary was actual intelligence and not disinformation.

No one knows for sure where the 35 pages of disinformation came from.

An ex-MI6 agent was supposedly hired to do opposition research on Trump by both Republicans and later Democrats.

He says he got the stuff from Russian sources.

And we already know that John McCain is the one who gave the memo to the FBI (not the CIA).
He admitted that.

Trump said in his press conference today that the memo came from a group of his opponents.
